Please join us Thursday, February 23rd, 2012 for the 2nd annual SOPH Tour of the Centers for Disease Control and Prevention! 

The trip will include:

• Attendance at the Advisory Committee for Immunization Practices (ACIP) Meeting

• An outside tour of the CDC Main Campus

• Lunch at the staff cafeteria

• A discussion with CDC Staff

• A visit and tour of the Global Odyssey Museum

The deadline for registration in February 6th, 2012. Both US citizens and non-citizens welcome. Please use the following form to reserve your seat on the trip. We will meet at the Ryals building and head to Atlanta no later than 6:00am. More details about transportation to follow.

First, please follow THIS LINK to register for the ACIP Meeting. When providing your information for registration, your organization is "University of Alabama at Birmingham School of Public Health". Please provide your LOCAL home address and your school email address. A parking pass (with security clearance to be on the grounds of CDC) will be mailed to the address you provide upon registration. International students will need to complete additional forms that will be emailed to you immediately by the CDC. Registration to the meeting is free. Printed email confirmation and any other items received from CDC (parking pass) are proof of registration. Bring these documents, a government issued photo ID (US CItizens), and/or a national passport (Non-US Citizens) with you on February 23rd, as they are required to attend the event with SOPH.
