    What is a completed application for admission?
        A Completed Application is:
 
 
   A Completed Application to the School of Public Health Consists Of:  

  • Completed Application Form
    MPH, MSPH, and DrPH
    must apply online to the centralized application service, SOPHAS, at www.sophas.org   PhD, MS and dual degree applicants must apply online at www.uab.edu/graduate
  • A non-refundable processing fee
    NOTE:
    Applications will not be processed until all required documentation and the fee has been received.  Applicants are required to pay an application fee any time an application is filed.  Applicants who have been denied admission and seek admission to the same or any other program at a later time will be required to pay the application fee again.
  • Two copies of official transcripts must be submitted from all institutions attended after high school
    MPH, MSPH, and DrPH –
    Official transcripts from every institution attended showing degrees awarded and date awarded must be submitted to SOPHAS, P.O. Box 9111, Watertown, Massachusetts 02471.  Domestic applicants must provide a final official transcript to the UAB Graduate School, 1530 3rd Avenue South, Hill University Center (HUC) Suite 511, Birmingham, Alabama  35294-1150 upon matriculation.  Foreign applicants must provide official transcripts and degree certificates, including English translation, if necessary, to the Graduate School upon matriculation. Failure to disclose all course work and/or degrees will result in the rescinding of the offer of admission.  Make sure that the name on the transcripts matches that of your application.  PhD, MS & dual degree applicants must send official transcripts to the UAB Graduate School, 1530 3rd Avenue South, Hill University Center (HUC) Suite 511, Birmingham, Alabama  35294-1150.  Failure to disclose all course work and/or degrees will result in the rescinding of the offer of admission.  Documentation not in English must have a translation attached.  Make sure that the name on the transcripts matches that of your application.
  • Three References 
    MPH, MSPH, and DrPH
    – References from former instructors, employers or other professionals who are competent to evaluate your potential to complete graduate study should be submitted online at www.sophas.org or mailed directly to SOPHAS, P.O. Box 9111 Watertown, Massachusetts  02471.  Make sure that your name on the recommendation form/letter matches that of your application.  PhD, MS & dual degree – References should be submitted online at www.uab.edu/graduate or mailed directly to the UAB School of Public Health, 1530 3rd Avenue South, RPHB 130, Birmingham, Alabama  35294-0022.  Make sure that your name on the recommendation form/letter matches that of your application.
  • Statement of Career Goals and Objectives
  • Resume or Curriculum Vitae (CV)
  • An acceptable admission test  - (scores must be sent from the testing centers)
    Please refer to the "General Admission Requirements Sections"  for detailed information on acceptable admission tests and the "Testing Information Section" for reporting information.
  • Test of English as a Foreign Language (TOEFL) -(scores must be sent from testing center)
    If your native language is not English or you did not receive a degree from an English-speaking institution, you must provide an official TOEFL with Test of Written English (TWE) score report. Refer to the "Testing Information Section"for reporting information. 
  • Affidavit of Financial Support (International Students Only)
    The U.S. Immigration and Naturalization Service
    requires the university to receive documentary proof of adequate financial resources to meet your expenses while at UAB.  This documentation must be received before the Office of International Scholars and Student Services (ISSS) will certify your visa eligibility.     Note that while the affidavit of support will not preclude your application for admission from being processed, it is important that the affidavit is received promptly to avoid delays in visa certification.
  • The applicant is responsible for making sure that the appropriate credentials are received by the appropriate entity by the stated deadlines.  All application materials become the property of the university and may not be released to any other individual or the applicant.
